I bill the following insurances for routine eye examinations.: Please verify with your insurance that you have this benefit available. Many health insurers subcontract routine eye exams out to secondary vision insurance companies such as Eyemed, Davis Vision, or VSP. (They don’t always tell you this!) Please note: I cannot bill any Medicaid or Apple Health Plans including Community Health Plan of WA, Molina, and United Health Care Community Plan. Special Note: 6-1-24- Although I have no plans to retire at this time I will be transitioning away from taking most routine vision insurance plans over the next year. At that time you will need to pay for your routine eye exam at the time of service. Most plans reimburse for out of network providers. I do plan to continue to accept medical insurance for specific medical eye problems although some plans require a referral from your primary care provider. All HMO plans require a referral.
Note: If your insurance provider is not listed on this page, full payment is due at the time of service. I accept Visa, Master Card, Discover, American Express, Checks, or Cash. I can give a reimbursement form to you for any insurance that I cannot bill. Check with your insurance to find out what they will reimburse if you see an out of network provider.
- Aetna: I will no longer be accepting Aetna Vision Plans starting January 1st, 2025. Please schedule your your yearly exams for 2024 as early as possible. There will not be enough times for everybody if you wait until December.

AARP Medicare Complete Medicare Advantage Plan by United Health Care. (Same as Spectera Vision) I will no longer be accepting this plan starting January 1st 2026.
- Amazon Vision Plan uses VSP. I am out of network. Payment required at time of service but I can bill it for reimbursement if allowed by your plan.

Boeing (Davis Vision) (Please note: Boeing Davis Vision does not cover contact lens fittings & evaluations. I will no longer be accepting this plan starting January 1st 2026.
- Cigna – Same as VSP. I am out of Network. Payment required at time of service but can bill it for your reimbursement if allowed by your plan.

- Humana – I will no longer be accepting Humana Vision Plans starting January 1st, 2025. Schedule your routine exams for 2024 as early as possible. There will not be enough times available if you wait until December.
- Kaiser (I can take most but not all PPO First Choice Plans) I cannot take standard Kaiser Plans. I will no longer be accepting any Kaiser plans after January 1st, 2025
- Met Life – Same as VSP. I am out of network. Payment required at time of service but I can bill it for reimbursement if allowed by your plan.

Premera Blue Cross (Please note: Even though we are a regular Premera provider I am not a Premera Medicare Provider.) Also Premera now contracts out most of their routine eye care plans to VSP. See VSP. Contact Premera or your HR Dept.about specifics of your plan.

Regence Blue Shield: Please note that Regence now contracts out most of their routine eye care plans including medicare advantage plans and Uniform Medical Plan to VSP. See VSP. Contact Regence or your HR department about the specifics of your plan. I am out of network for VSP. Contact VSP to make sure they will reimburse you if you see an out of network provider.
